If you are registering online you will still need to send in the required two copies of your player's CYSA Form 1601. Please use the link provided during your on-line registration session. This will produce a data-populated form, which only requires a signature in the "Medical Liability and Release" box. If for some reason you no longer have that form available, then print two copies of a regular blank CYSA Form 1601 (same as above), fill them out, sign them and mail them in. Our parent body CYSA does require actual wet ink signatures. Once signed, the forms must be sent in by mail to: Registrar, POB 1992, Sonoma, CA 95476, along with the player's birth certificate (if they have not played with us in the last two years) and payment (if paying by check). |
