Once you have completed the online form, please email your confirmation number to registrar@sonomasoccer.org.

FIRST TIME COACHES, MANAGERS, AND VOLUNTEERS

If this is your first time filling out the Adult Staff Disclosure Form 1628, please send two copies of the completed form.  Please do not fax or scan and email these forms. We need wet ink signatures and we need to see the actual driver's license. If mailing in, please enclose one copy of your driver's license (we may check the actual document later). Please note that in addition to the two copies of CYSA form 1628, SVYSA Risk Management has a policy requiring fingerprinting and background checks.
