The Adult Staff Disclosure form is required from all coaches, assistant coaches, team managers, and any other adults working with children or handling paperwork for the League.
RETURNING COACHES, MANAGERS, AND VOLUNTEERS If you have filled out this form previously, you can now fill out your 1628 Adult Staff Disclosure Form online using the following link: CYSA Adult Online DisclosureOnce you have completed the online form, please email your confirmation number to registrar@sonomasoccer.org. FIRST TIME COACHES, MANAGERS, AND VOLUNTEERS
If this is your first time filling out the Adult Staff Disclosure Form 1628, please send two copies of the completed form. Please do not fax or scan and email these forms. We need wet ink signatures and we need to see the actual driver's license. If mailing in, please enclose one copy of your driver's license (we may check the actual document later). Please note that in addition to the two copies of CYSA form 1628, SVYSA Risk Management has a policy requiring fingerprinting and background checks.

A quick note to all of those interested in coaching a team with SVYSA in the coming year. In keeping with our Risk Management Policy, the SVYSA arranges and pays for a mandatory fingerprinting and background check for all new coaches. Children's safety is a top priority for the SVYSA and we feel these procedures help maintain the integrity and security of our League, our coaches and, most importantly, our children. This is a friendly reminder for all Coaches, Assistant Coaches, Managers, Trainers, and Parents on Traveling Teams:
- If you are transporting players, be sure you have their parent’s permission in advance, and that you have a valid CA driver’s license with current insurance.
- It is recommended you not share a hotel room with any player other than your own child. If for some reason you share lodging with other players, you’ll want to obtain a written permission note from any involved parent(s).
