This form is required from all coaches, assistant coaches, team managers, and any other adults working with children or handling paperwork for the League. Please do not fax this in. We need wet signatures and we need to see the actual driver's license. If mailing in, please enclose a copy of your driver's license (we may check the actual document later), and note the SVYSA Risk Management policies requiring fingerprinting and background checks.PDF Document
